The story begins with two American tourists, Lindsay (Ashley C. Williams) and Jenny (Ashlynn Yennie), driving through a German forest. After their car suffers a puncture, they find themselves stranded in the woods. Seeking help, they stumble upon a remote villa belonging to a retired surgeon, Dr. Josef Heiter. Unfortunately for them, Dr. Heiter is not the savior they hoped for. A third victim, a Japanese businessman named Katsuro (Akihiro Kitamura), is also captured. The deranged doctor has a singular, macabre obsession. Formerly world-famous for separating conjoined twins, he now wishes to go in the opposite direction: to connect humans in a chain, surgically joining the mouth of one person to the anus of another to create a shared digestive system—the titular "Human Centipede".
